DX Stage 1A <.3mm, Grade 3, Very High Ki-67. ER+/PR+/Hert2-, Neg Nodes. If So What Treatment Plan Did You Choose?

I was originally diagnosed with DCIS R breast focal location. Lumpectomy performed and in the lumpectomy a very small 2.5mm grade 3 tumor was found Ki-67 90%. ER+/PR+/Hert2 -. Second surgery tested sentinal node that was negative and removed more tissue for very large margins. Oncotype may not be able to be performed and even if so may not give accurate results due to small size. Smallest recommended sample 6mm to do the test 3 times for accuracy. Yeah this is tough. The oncologists don't agree because there is no concesus on this. It's small, it's aggressive, there are no nodes positive, and you are ER positive, so technically you could get away with surgery and anti estrogen meds and no chemo. But I am biased and would take the chemo because it makes me feel more comfortable. It's not rational or even medical, it's about your comfort level. Third opinion is good, make sure you see an onc that specializes in breast cancer!

I know how hard it is to make the decision and think about it. I was told it was up to me. I found that my PR was neg and the dr said that nobody knew the significance of the neg PR. When looking up on the internet I found some research that said that the neg PR made it harder for the hormone blockers to work. That was my choice right there. I did what I needed to do to make sure that I had done everything right. Good luck with your decision. I did post my question too to get everything I could before telling the onco that I was going to do the chemo. BIG HUGS

I had a 9mm tumor and Ki-67 of 44%, no cancer in the nodes. My onc is putting me on 4 rounds of Taxol because of the high proliferation.

Life would be easier if the doctors would at least agree. Mine was IDC 2.3cm grade 3 90% ER+ PR+ HER2-. They never found any cancer in my breasts, it was only in 1 lymph node. But being in my lymph node there was no questions, I needed chemo. Glad you are getting a third opinion. Best of luck on your decision.

Hi @A MyBCTeam Member, I was originally diagnosed with DCIS as my tumor was very small (I am trying to remember off-hand but I believe it was less than 1cm). I was then diagnosed with IDC (I had a small invasion into the breast) but had clear margins due to my bilateral mastectomy. However, my sentinel nodes had isolated tumor cells within them which is why chemotherapy was recommended. However, it really depends on your age. I am not sure how old you are. I was 23 when diagnosed. Did you have clear margins with the lumpectomy? If so, and you are node-negative then I know it is more of a personal decision. I am glad you are going for multiple opinions. Unfortunately, you are in a situation where you have to go with what you think is best for you. If you are young, then I would probably say go the chemotherapy route. But I am not a doctor, and I am biased. I just wanted you to know that someone out there was listening and I wish you the best of luck in your decision making!
