It can and it spreads differently, either locally , through the lymph nodes or blood stream your doctor will likley start you on meds based on the type of cancer you have

I don't know how my Breast tumor spread but it looks like it skipped the lymph nodes altogether and Mets to sternum. 1 out of 4 Sentinel nodes tested positive but there was already a large lytic lesion on my sternum.
