What Are The Best Treatment Options Available For Early Stage Triple Negative (TNBC) In 2023?

A MyBCTeam Member asked a question 💭
Cleveland, OH

I have just had surgery (unilateral mastectomy) and hopefully will get my pathology report in a few days. Of course, I don't know what it is going to say! But I'd like to have some idea of both what options may be recommended as well as what other women's experiences with them have been. I have intraductal invasive carcinoma and was graded 1B prior to surgery. I know that no one can tell me what's to come but I'd like to go into my appointment educated. Thanks.

For TNBC, they usually recommend Neo adjuvant (before surgery) chemotherapy, because it such an aggressive cancer. I was diagnosed 2018, had AC and Taxol, followed by surgery and radiation. I was very lucky to get complete pathological response to chemo, no cancer was left by surgery time. I was stage as well, IDC.
Current standard of care now adds Carboplatin and Keytruda.
TNBC has a very high risk of recurrence in the first 2-3 years.
Honestly, if chemo isn’t in the discussion, you may want a second opinion.
Wishing you the best!

You are correct that onco scores are only for hormone positive cancers.

For early-stage triple-negative breast cancer (TNBC), the treatment options typically include:

- Chemotherapy: Often the main treatment, sometimes combined with an immunotherapy drug like pembrolizumab (Keytruda). Chemotherapy can be given
- Neoadjuvant therapy: Before surgery to shrink the tumor.
- Adjuvant therapy: Show Full Answer

- Adjuvant therapy: After surgery to eliminate any remaining cancer cells.

- Surgery: To remove the tumor.

- Radiation therapy: May be used in combination with other treatments.

The specific treatment plan depends on the disease stage and whether the cancer has relapsed.

Yes Mary, currently no evidence of cancer, TNBC is a real jerk!
I think I will feel better when I hit my 5 years from treatment next year,
Hoping you are doing well on this journey!

From what you wrote, I take it that you haven't had any recurrences? Hope that's the case!
One (of many) interesting things about making decisions and getting recommendations about our cancer is that we will never know what would have happened had we taken a different route. Did we do more than necessary? Were we not aggressive enough? Or was whatever happened what was going to happen regardless? All we can do, of course, is make our best decision once we have opinions that make sense to us.
