Need Input On Surgical Decision -- DCIS High Grade

Nearly 67 years old. No family history of cancer.

Routine mammogram in May showed microcalcifications classified as suspicious. Expectation set of potential lumpectomy since suspicious area 1.5cm. Biopsy indicated high grade, comeda, hormone receptor positive. Follow up mammogram indicated 6.5cm. Discussion turned to masectomy. MRI w-contrast ordered given inconsistency. It showed 2cm. I thought Sentinnel Node Biopsies were standard with all breast cancer surgeries. My case was a little different. I chose a BMX since I was 66 and my mammogram just showed one 2.1 cm IDC tumor,grade 3. So they only injected that one tumor with the Sentinnel dye. When my pathology came back there were 4 separate tumors in my removed breast with cancer in between. So they did not test all the tumors and each tumor can lead to different Sentinnel nodes. So the Tumor Board wanted me back in surgery to remove all of my axillary nodes which would definitely put me at risk for Lymphdemia. My breast surgeon disagreed with the tumor board. So we just did CT and Bine scans which did not show anything. But I always will worry. I also did chemo and radiation to kill anything that was there,

I am 66 and had a 2.2 cm tumor
high grade . They did a lumpetomy and waited until results of Taylor’ X study were out. I just completed radiation and will start 8 rounds of chemo
Radiation was easy . The only side effect I had was fatigue .

I highly recommend a second opinion, preferably from a major cancer center at a teaching

I was able to have what is called direct to implant, during my BMX, what this is the breast surgeon removes the breast tissue and then the PS steps in and does the implants no expanders. But you have to be a candidate and not every surgeon performs direct to implant. This really worked for me, and I would recommend this this surgery for good candidates. Hope that helps whatever the decision will be right for you.
